Application Materials

Complete and return your application to Denver JDS today.

The admission application will be available starting November 1, 2016. Please note that all applications materials, along with a non-refundable $100 application fee, must be received by February 10, 2017 in order to to be considered for admission. Applications received post-deadline will be considered on a space-available basis.

 

Forms that will be required:

Kindergarten

  • General Family Information Form
  • Biographical Sketch (completed by the parent)
  • Confidential Teacher Recommendation Form (parent provides blank form to the teacher, who will return it to our admission office)

Grades 1-5

  • General Family Information Form
  • Biographical Sketch (completed by the parent)
  • 1st Grade Teacher Recommendation Form  (parent provides blank form to the teacher, who will return it to our admission office)
  • 2nd-5th Grade Teacher Recommendation Form
    (parent provides blank form to the teacher, who will return it to our admission office)
  • Signed Request for School Records

Grades 6-12

  • General Family Information Form
  • Biographical Sketch (completed by the parent)
  • Student Statement (completed by the student)
  • Transcript Release and Confidentiality Form
  • Math Teacher Recommendation Form (student provides blank form to the teacher, who will return it to our admission office)
  • English Teacher Recommendation Form (student provides blank form to the teacher, who will return it to our admission office)
  • Additional Teacher Recommendation Form (student provides blank form to the teacher, who will return it to our admission office)
  • Current Principal, Counselor or Advisor Form (student provides blank form to the teacher, who will return it to our admission office)
  • Judaic Teacher Recommendation Form (if applicable, student provides blank form to the teacher, who will return it to our admission office)
