The following items, along with a non-refundable $100 application fee, must be received  by January 10, 2011 for applications to be considered complete:

Kindergarten

General Family Information Form (can be filled out electronically)
Biographical Sketch
Confidential Teacher Recommendation Form (parent will provide blank form to the teacher who will send it to the Admission Office)

Grades 1 through 5

General Family Information Form (can be filled out electronically)
Biographical Sketch
Teacher Recommendation Form1st Grade (parent will provide blank form to the teacher who will send it to the admission office)
Teacher Recommendation Form 2nd-5th Grade
Signed Request for School Records

Grades 6 through 12

Transcript Release and Confidentiality Form
Biographical Sketch (completed by the parent)
